I was pleased to note this morning (April 14, 2010) that the New York Times reported on the Agency for Healthcare Research and Quality Report released yesterday which noted HOSPITAL ACQUIRED INFECTIONS NEED URGENT ATTENTION.

The Report focused on Hospital Acquired Infections, noting that the, “problem of hospital-acquired infections remains as resistant to cure as the germs that contribute to an estimated 100,000 deaths a year, according to an annual government study issued Tuesday.”

Despite threats of governmental sanctions, hospitals continue to see increased rates of post-operative bloodstream infections which increased by eight percent and catheter-associated urinary tract infections which increased by four percent.

There was no change in the incidence of bloodstream infections caused by the placement of catheters in central veins. The only positive news came from a 12 percent reduction in the rate of post-operative pneumonia.

Dr. Carolyn M. Clancy, the agency’s director, pointed out that projects across the country had shown remarkable success in reducing infection rates by adhering to basic standards for hand hygiene, disinfection of patients, sterile handling of equipment and proper use of antibiotics. But at many hospitals those successes have yet to overcome an entrenched medical culture.

‘Kathleen Sebelius, the secretary of health and human services, called the trend troubling but said the new health care law would ”help turn these numbers around.” Under the law, hospitals with high rates of infections will be penalized by the government starting in the 2015 fiscal year.

I want to take a closer look at what the new Healthcare Law does in terms of hospital acquired infections — and why Medizone International’s successful decontamination by AsepticSure™ to the 6 log standard or greater on hospital surfaces represents solution for Hospital Administrators in terms of revenue.

Consumers Union, a nonprofit group that publishes Consumer Reports, analyzed the patient safety initiatives in the health reform law.  Health reform law addresses issues associated with Medicare patients.  Because Medicare patients represent half of all hospital patients hospitals will be paying attention.

Medicare policies tend to dictate a hospital’s practice for all patients.  Private insurers and other public health programs change their policies to follow Medicare.

Medicare’s Hospital Compare Web site will publish individual hospital errors rates for about a dozen avoidable conditions for Medicare Patients including hospital acquired infections.  This is the same list of conditions that Medicare stopped paying for in 2008.

Medicare payments to hospitals hospitals will be adjusted based on patient outcome.

Hospitals with better outcomes for pneumonia and preventing infections will benefit from higher payments.

Hospitals with high rates of hospital-acquired infections receive reduced payments.

For example, when Medicare stopped paying for catheter-related urinary tract infections — forcing the hospital to bear the cost on their own — many hospitals implemented system-wide practices to prevent infections. Techniques included better tracking of patients with catheters and automatic messages to remind doctors to remove catheters because the longer the device is in place, the greater the chance for infection.

Dr. James Conway, a senior fellow at the Institute for Healthcare Improvement in Boston, said the law raises the bar for quality.

Beginning in 2012, hospitals that meet or exceed performance standards in conditions including stroke care, heart attack outcomes and infections will receive higher Medicare payments.

New Healthcare Law:

Penalizes hospitals with high error rates: In 2014, Medicare will cut pay by 1 percent to hospitals with the highest rates of medical harm.

Requires public reporting of medical injuries: In 2014, the Medicare web site Hospital Compare will post each hospital’s record for certain medical errors and hospital-acquired infections.

Insurers to demand better care: By 2012, insurers must have contracts with medical providers that include actions to reduce readmission, improve safety and use best practices.

Emphasizes effective practices: The law creates a Patient-Centered Outcomes Research Institute run by a private foundation that will find the most effective drugs, medical devices and procedures.

Establishes a patient safety center: The Center for Quality Improvement and Patient Safety will research and disseminate information about improving the quality and safety of health care, with an initial focus on preventing infections and reducing re-admissions.

Requires reporting of gifts given to doctors: In 2013, device and drug manufacturers must begin reporting details about gifts and payments over $10 to doctors and teaching hospitals. The information will be published on a searchable Web site.

Improves quality measures: The law allocates $75 million annually toward improving the measures used to evaluate quality and safety.

While MRSA is the ’superbug’ obtaining the most press recently —it is important to remember this is not the only ’superbug’ AsepticSure™ kills and not the only ’superbug’ hospital administrators worry about.

Medicare will begin implementing cost controls on health care providers, mostly through penalties and incentives and reduce payments for hospital-acquired infections or preventable hospital admissions.  Hospitals are scrambling for solutions.

AsepticSure™ can reliably achieve 6 log (99.9999%) kill or greater against Pseudomonas Aeruginous.

Pseudomonas Aeruginous is also a big problem for hospitals and responsible for as many as one in ten hospital acquired infections. Disinfectants commonly used in homes and medical facilities can boost the resistance of some bacteria to life-saving antibiotics, according to findings published in the journal Microbiology which also pointed to the possibility that other drug-resistant hospital killers — such as MRSA (methicillin-resistant Staphylococcus aureus) and C-diff (Clostridium difficile) — could spread and build up immunity to antibiotics in the same way.

The findings shed light on how Pseudomonas aeruginosa spreads.

P. aeruginosa is a an “opportunistic” bacteria that attacks people with weakened immune systems.

It typically infects the pulmonary and urinary tracts, as well as burns and puncture wounds.

In laboratory experiments, researchers showed that the bug can rapidly mutate, building resistance to progressively higher doses of a disinfectant known as BSK, or benzalkonium chloride.

Safe for humans, BSK is widely-used in cleaning and disinfecting products to kill bacteria, fungi and algae.

The DNA-altered bacteria were able withstand concentrations of BSK up to 400 times greater than the non-mutated strain.

More critically, they also developed a resistance to an antibiotic, ciprofloxacin, even though they had never been exposed to the drug.

Ciprofloxacin is a front-line medication in the fight against several bacterial infections, and is also the drug of last-resort against the deadly disease anthrax.

The disinfectant-resistant strain of P. aeruginosa built up immunity against ciprofloxacin up to ten times more effectively than did the baseline bacteria, the study reported.

In further experiments, Fleming and colleagues put the two strains together in an environment containing a diluted dose of disinfectant, such as might be found in a hospital or home.

The mutated bugs were “highly competitive” with the non-mutated ones, noted the report’s author, “They outgrew the so-called ’sensitive’ strains so rapidly it was hard to believe.”

“That means that we have a problem — disinfectant may proliferate antibiotic resistance,” he added.

Medizone has assembled an international team of professional engineers who are finalizing design of their first pre-manufacturing prototype to be used in hospital beta testing, which will commence later this spring.   This time is getting close and it is important to watch for announced results.

There are so many reasons to be excited about the AsepticSure™ hospital sterilization system — and here is another one.  No matter how hard a cleaning crew tries to clean a hospital room MRSA may linger behind.  Hospitals MUST get a handle on infections.  Medicare no longer pays for Hospital Acquired Infection treatment.   Private Insurance is following suit.  State Legislatures are jumping on board.  The recent Duke University study which put the cost to treat an MRSA infection at $60,000 per patient put this problem in perspective.
